Live from the day you sign up
- A form called Personal Training Enquiry is ready under Marketing → Forms. Copy the link and put it in your bio, your website or a message.
- It carries your logo, banner, colours and font from Settings → Branding.
- Add questions in short text, long text, number, single or multiple choice, and yes or no.
Advanced forms, one page at a time
- A progress bar, and text, a picture or a video between questions.
- Show a page only when an earlier answer matches: a rule, or several, per page.
- Starts with three pages, name, contact details and goals, which you can rearrange.
PAR-Q as a link, not a PDF
- From the lead’s record, click Send PAR-Q to email a branded link, or copy it to send yourself.
- The pill reads PAR-Q Done when they finish, or PAR-Q Review if any answer needs a look.
- Their answers carry across to the client record when they sign.
What happens after they press submit
- The lead appears in your pipeline as New, with the form as the source and their answers under Goals.
- You get an in-app notification and an email.
- Turn on the New enquiry welcome automation and they get a reply from you within moments.

Yes. It is a link, so it goes anywhere a link goes: a button on your site, your Instagram bio, your Google Business profile or a direct message.
Can I have more than one form?
Yes. Make one per offer or audience, basic or advanced. Each has its own link.
Does the prospect get a reply?
The form itself sends nothing. Turn on the New enquiry welcome automation and a reply goes out from your name straight away.
What is in the PAR-Q?
The standard physical activity readiness questions. Yes answers are flagged on the lead so you see them before the first session.
